Summary
Molecular hydrogen (H2) is a physiologically inactive gas. However, over the past 10 years, an ever-increasing body of evidence has demonstrated the biological functions of hydrogen under pathological conditions. More specifically, H2 has protective properties against several diseases, particularly nervous system disorders, including ischemia/reperfusion injury, trauma, subarachnoid haemorrhage, nerve pain, neurodegenerative diseases, cognitive dysfunction induced by surgery and anaesthesia, anxiety, and depression. In addition, H2 possesses multiple protective properties primarily through antioxidation, anti-inflammation, anti-apoptosis, regulation of autophagy, and preservation of mitochondrial function and the blood-brain barrier (BBB). Furthermore, H2 is easy to use, and its neuroprotective effect is achieved without major side effects. This indicates that the administration of H2 could potentially constitute a treatment strategy. Here, we summarize the H2 donors and their pharmacokinetics. Concurrently, we review the efficacy and safety of H2 as a treatment for various diseases of the nervous system based on preclinical and clinical studies. This leads to the conclusion that H2 may be a simple and effective clinical treatment for central nervous system (CNS) diseases, such as cerebral ischemia/reperfusion injuries, Parkinson's, and diseases characterized by cognitive dysfunction. In addition, the mechanisms potentially involved in the protective effect of H2 are analysed.

Introduction

Molekylær brint (H2) blev opdaget af kemikeren Henry Cavendish i 1766. Det er en farveløs, lugtfri og fysiologisk inaktiv luftart. De biologiske funktioner af H2 blev gradvist påvist af forskerne i slutningen af det 20. århundrede. I 1975 opdagede Dole [1], at hyberbar H2 (2,5 % O2 og 97,5 % H2) forårsager regression af pladeepitelkarcinom hos hårløse albinomus. Han opstillede en hypotese om, effekten af H2 muligvis kan tilskrives gassens evne til at regulere den meste skadelige OH-radikal (oxidanthydroxyl). I 2001 opdagede Gharib [2], at 0,7 MPa H2 har en antiinflammatorisk effekt på kronisk leverinflammation, der skyldes schistosomiasis. Flere år senere, i 2007, påviste Shigeo Ohta, at H2 har den samme beskyttende effekt som antioxidanter i hjernevæv, der udsættes for oxidativt stress, ved selektivt at regulere den cytotoksiske frie radikal OH og peroxynitrit (ONOO-) [3]. Siden da har disse resultater udløst et antal studier, hvor man har undersøgt den potentielle beskyttende effekt af H2 i forhold til flere sygdomme og de relaterede molekylære mekanismer.

Sygdomme, der skyldes abnormalitet i nervesystemets struktur og funktion, er normalt meget voldsomme sygdomme med høj incidens af invaliditet og mortalitet over hele verden. Genoprettelse af den neurale struktur og funktion efter enten akut skade eller kronisk neurodegeneration er stadig en udfordring, da der ikke findes effektive helbredelsesmuligheder til de fleste neurologiske sygdomme, og sygdommenes reaktion på traditionel intervention såsom medicin, fysioterapi, neurorehabilitation og forebyggende tiltag er ikke god. I modsætning hertil har stadig mere evidens vist, at H2 har en beskyttende effekt i forhold til forskellige neurologiske sygdomme, herunder iskæmi/reperfusionsskade [4–6], traume [7, 8], subarachnoidalblødning (SAH) [9, 10], nervesmerter [11, 12], Alzheimers [13, 14], Parkinsons [15, 16], stemningsforstyrrelser [17, 18], glioblastom [19] og hjerneinfarkt [20]. Til dato er der udført mere end 60 kliniske forsøg vedrørende brugen af H2 til mange sygdomme, der involverer flere systemer, og mindst 10 af forsøgene omhandlede nervesystemsygdomme, herunder akut cerebral iskæmi [4], akut hjerneinfarkt [20], følger efter hjertestop [21, 22], Parkinsons [23, 24] og stemningsforstyrrelser
[18].

There are a number of advantages to H2 as a neuroprotective gas. First, the gas can cross the blood-brain barrier (BBB), penetrate bio membranes, and diffuse into the cytosol and organelles [3]. Additionally, no major side effects have been documented [4]. More importantly, repeated administration of H2 does not cause intolerance [25]. Furthermore, several easy-to-use and practical administration options are available [26, 27]. Finally, H2 has a protective effect against numerous diseases, including diseases of the peripheral nervous system and the central nervous system (CNS) [26, 28]. In Japan, for example, inhalation of 2% H2 has been approved for emergency clinical treatment of cardiac arrest. This means that H2 is a novel and potential therapeutic strategy for the prevention and treatment of numerous diseases, including neurological diseases. The advantages derived from H2 have further led to the development of the H2 health industry.

I denne gennemgang er det vores mål at opsummere den aktuelle viden om den neurobeskyttende effekt af H2 i forhold til forskellige nervesystemsygdomme (fig. 1) og de mekanismer, der muligvis indgår (tabel 1).

H2 donors

Aktuelt er der adskillige metoder til administration af H2, herunder inhalation af H2-gas, indtagelse af H2-beriget vand, injektion af H2-beriget saltvand, bade i vand med H2, indtagelse af en fast H2-bærer (koral calciumhydrid) og H2-producerende forløbere (f.eks. laktulose og L-arabinose) [27, 65–67], samt funktionelle mikro-/nanomaterialer til målrettet levering af H2 [68].

I klinisk praksis omfatter de almindelige administrationsmetoder for H2 primært inhalation af H2-gas, indtagelse af brintberiget vand, injektion af brintberiget saltvand og bade i H2-vand. Inhalation af H2 er den nemmeste og mest direkte metode, og den oftest anvendte koncentration er 1 % – 4 % (sikker koncentration). I sammenligning med inhalation af H2-gas er indtagelse af brintberiget vand mere sikkert og mere praktisk. Brintberiget saltvand indgives normalt via IV eller intraperitoneal injektion. Brintbade anvendes ofte til behandling af hudsygdomme [69]. Når H2 administreres via indtagelse af brintberiget vand, frigives 59 % af det nedsvælgede H2 i åndedrættet, ca. 0,1 % via kroppens overflade og ca. 40 % optages i kroppen [70]. Efter frivilliges indtagelse af 500 ml brintberiget vand på under 1 min. nåede koncentrationen af H2 i åndedrættet et peak-niveau på ca. 36 ppm efter 10 min., hvorefter det gradvist faldt til baseline-niveau på ca. 7,0 ppm efter 60 min. [70]. Ono et al. opdagede, at koncentrationen af H2 i både arterie- og veneblod hurtigt stiger og når et plateauniveau (10 lmol/l til 20 lmol/l) på 20 min. efter opstart af 3 % og 4 % H2. Ved seponering af inhalation af H2 falder koncentrationen i arterieblod til <10 % af plateauniveau på ca. 6 min., men på ca. 18 min. i veneblod [4]. Under 30 min. IV infusion af brintberiget saltvand (0,8 mmol/l) stiger koncentrationen af H2 hurtigt i både arterie- og veneblod til maks. (< 1,8 ppm) efter ca. 15 min., og falder hurtigt ved seponering af infusion af brintberiget saltvand [71]. Generelt fører inhalation til en højere koncentration af H2 i blodet end IV infusion [71]. Dyreforsøg har endvidere vist, at inhalation inducerer højere koncentrationer af H2 i hjernen end andre metoder til administration af H2 (oral, intraperitoneal, intravenøs) [72]. Disse resultater antyder, at inhalation er den foretrukne administrationsform til behandling med H2 ved CNS-sygdom.

Selvom inhalation, oral indtagelse og injektion af H2 på effektiv vis dæmper sygdomme i nervesystemet, sådan som det er påvist af forskellige forskningsgrupper [6, 20, 73], mangler der sammenligninger af de biologiske effekter af de forskellige brintinterventionsmetoder i forbindelse med en bestemt sygdom. Da koncentrationen af H2 i væv og organer er meget forskellig i overensstemmelse med den valgte intervention [72, 74], kan administration ved hjælp af forskellige metoder have forskellig virkning i det samme beskadigede væv. Hertil kommer, at kun få studier har fokuseret på dosis- og tidsafhængige effekter og tolerance overfor H2 i grundlæggende og kliniske studier. For at vælge den mest effektive behandlingsmetode med H2 for hver sygdom er det derfor vigtigt at opnå yderligere forståelse for farmakokinetikken og terapeutisk virkning af de forskellige H2-donortyper. I musemodeller for Alzheimers reducerer intracerebral injektion af nanopartikler med Pd-hydrid (PdH) overgenereringen af amyloid beta (Ab) i hjernen [14], mens indtagelse af brintberiget vand ikke udviser den samme effekt [13]. Opløseligheden af H2 er lav, og gassens koncentration i musehjerner er væsentligt lavere (< 30 ppb/g) ved traditionel administration (indtagelse af brintberiget vand, injektion af brintberiget saltvand og inhalation of 4 % H2) [72]. PdH-nanopartiklerne, som er et H2-opbevaringsmateriale med høj nyttelast, frigiver bæredygtigt ca. 6 lmol/l H2 indenfor 60 timer [14, 75]. Dette fænomen har indikeret, at Ab clearance muligvis korrelerer med koncentrationen og varigheden af H2 i hjernen. Det vil sige, at en høj koncentration af H2 i målvævet har en bedre performance end en lav koncentration. Med henblik på maksimering af den terapeutiske effekt af H2 kan udvikling af effektiv opbevaring og målrettet levering af H2 være en af de fremtidige forskningsretninger.

The protective effect of H2 in relation to nervous system diseases

Iskæmi/reperfusionsskade

Iskæmi/reperfusionsskade er en tilstand karakteriseret af vævsskade, som er forårsaget af en iskæmisk eller anoxisk periode efterfulgt af genetablering af blodforsyning til vævet. Iskæmi/reperfusionsskade i centralnervesystemet forbindes med sygdomme som slagtilfælde, hjernetraume, hjerneinfarkt og hjertestop.

Genoprettelse af cirkulation efter fravær af tilstrækkelig tilførsel af O2 i blodet inducerer en eksplosion af reaktive oxygenarter (ROS), hvilket derefter udløser inflammatorisk respons og oxidativ skade. ROS-produktionen kan direkte ødelægge cellemembranerne ved at inducere lipidperoxidation, hvorfor antioxiderende midler anses for at være en behandlingsmulighed. For det første er H2 blevet undersøgt som et antioxiderende middel, der kan afbøde den destruktive effekt af oxidativt stress i hjernen efter fokal iskæmi/reperfusion via selektiv reducering af cytotoksiske oxygenradikaler [3]. Det udgør det grundlag, hvorpå in vivo-studier er blevet udført for at validere den beskyttende rolle, som H2 har i forbindelse med iskæmi/reperfusionsskade i centralnervesystemet. I dyremodeller for cerebral iskæmi/reperfusion øger inhalation af 66,7 % H2 i væsentlig grad aktiviteten af SOD og GSH-Px, reducerer niveauet af malondialdehyd (MDA) og infarktvolumen, dæmper ødem og blødning i hjernen samt forbedrer neuroadfærdsmæssige mangler [5, 76, 77]. Iskæmi/reperfusionsskade i hjernen er en almindelig sekundær effekt af hjertestop, som er ansvarlig for mortalitet og morbiditet efter hjertelungeredning. Ved eksperimental hjertestop/genoplivning mindsker H2-intervention også i væsentlig grad de neurologiske skader samt øger overlevelsesraten og det neurologiske resultat hos dyr [78, 79]. Inhalation eller injektion af H2 efter hjertestop kontrollerer effektivt neuronal død og mikrogliaaktivering i hippocampus samt reducerer niveauet af protein S100b i serum [80–82]. Hertil kommer, at det er påvist, at inhalation af H2 alene eller i kombination med terapeutisk hypotermi er bedre end hypotermi alene [38, 80, 81, 83].

Klinisk iskæmi/reperfusion forekommer ikke ofte i forbindelse med slagtilfælde og hjerneinfarkt, hvorimod der er større sandsynlighed for udvikling af de frygtede reperfusionsskader i hjernen hos patienter, som får trombolysebehandling eller interventionstrombektomi. Endnu mere vigtigt er det, at sikkerheden og virkningen af H2 er blevet bekræftet i kliniske forsøg med cerebral iskæmi, hjerneinfarkt og hjertestop. Hos patienter med akut cerebral iskæmi kan inhalation af 3 % H2 i 30 min. eller IV administration af H2 leverer tilstrækkeligt H2 til blodet uden at kompromittere sikkerheden [4, 84]. Et randomiseret, kontrolleret klinisk studie med patienter med akut hjerneinfarkt viste, at inhalation af 3 % H2-gas i 1 time 2 gange om dagen i 7 dage forbedrede O2-mætningen uden bivirkninger. Patienter, som behandles med inhalation af H2, har væsentligt mindre infarktsted, og oplever bedre opklaring af den neurologiske status og evnen til at udføre dagligdags aktiviteter i forhold til kontroller [20]. Hertil kommer, at IV administration af H2 i kombination med edaravon har mere evident og signifikant favorabel effekt end kun administration af edaravon [85]. I et humant studie med patienter med følger efter hjertestop havde inhalation af en lav koncentration af H2 i 18 timer en gunstig score for kategorien cerebral performance efter 90 dage, uden rapportering af bivirkninger [21]. Med henblik på yderligere evaluering af virkningen og sikkerheden af inhalation af H2 er man i Japan i gang med et fase II-, klinisk forsøg med patienter med følger af hjertestop [22]. Selvom der ikke er fundet bivirkninger af H2 i dyreforsøg, bør de potentielle bivirkninger undersøges yderligere, da der er indberettet diarré hos et lille antal patienter efter behandling med H2 [84].

Hypoksi/iskæmisk neonatal hjerneskade

Hypoksi/iskæmisk neonatal hjerneskade er en primær årsag til dødsfald og invaliditet i den perinatale periode, men der er ingen effektiv behandling. Nyere studier med gnavermodeller for neonatal hypoksi/iskæmi har dog vist, at H2 kan beskytte neonatale hjerner mod skader, der skyldes hypoksi/reoxygenering. Adskillige studier har påvist, at intraperitoneal injektion af brintberiget saltvand i væsentlig grad undertrykker autofagi og neuroinflammation, fremmer polarisering af M2-mikroglia, redder synaptisk tab og derefter genopretter adfærdsmæssige mangler hos en musemodel for hypoksi/iskæmi [39, 40]. Der er opnået lignede resultater med hypoksi/iskæmimodeller hos neonatale rotter og smågrise [37, 38]. I en rottemodel for neonatal hypoksisk-iskæmisk encefalopati er det påvist, at inhalation af H2 reducerer infarktstørrelsen, neuronalt tab og astrocytaktivering i cortex og CA3-regionen af hippocampus [37]. Sideløbende blev de adfærdsmæssige reflekser hos neonatale rotter væsentligt forbedret efter inhalation af H2 [86]. Et studie by Htun et al. viser, at H2-ventilation kombineret med mild hypotermi forbedrer den neurologiske score og gangfunktionen i en 5-dags hypoksi/iskæmi-model med neonatale smågrise [38]. H2 har ikke kun en kortvarig neurobeskyttende effekt, men også langvarige neurologiske og neuroadfærdsmæssige effekter. 10 uger efter den hypoksiske/iskæmiske irritation i en neonatal rottemodel forbedrer administration af H2 også læring og hukommelse [37]. Den neurovaskulære enhed er nødvendig for at opretholde hjernens skrøbelige homeostase. ROS produceret i den tidligere reoxygeneringsperiode reducerer i væsentlig grad cerebrovaskulær reaktivitet og inducerer dysfunktion i den neurovaskulære enhed. H2 bevarer cerebrovaskulær reaktivitet og dæmper udvikling og persistens af forsinket neurovaskulær dysfunktion forårsaget af hypoksisk stress hos nyfødte smågrise [41, 87]. Samlet set indikerer disse resultater, at brug af H2 kan anses for at være en terapeutisk tilgang til neonatale hjerneskade efter asfyksi.

Trauma

Incidensen af traumer i hjerne og rygmarv er konstant stigende i vore dages samfund. Selvom der er sket fremskridt inden for profylaktisk og terapeutisk behandling af traumer, er det stadig en stor udfordring af genoprette nervefunktionen. Hos dyr medfører traumer i hjerne og rygmarv blødning, ødem, celledød og inflammatorisk celleinfiltration samt forøger BBB-permeabilitet og neurologiske mangler. Behandling med H2 forbedrer dog i væsentlig grad skader og fremmer genopretning af nervefunktionen [7, 8, 42, 88, 89].

Treatment with H2 or hydrogen-rich saline reduces the expression of caspase-3, caspase-9, and Bax, increases the expression of Bcl-2, and significantly attenuates neuronal apoptosis following a mechanical injury [8, 42]. In addition, treatment with H2 also reduces oxidative products, e.g., MDA, 8-iso-prostaglandin F2a, 8-hydroxydeoxyguanosine (8-OHdG), and carbonyl protein, increases endogenous antioxidant enzyme activity (SOD, CAT, and GPx), suppresses the levels of MPO, NOX2, and NOX4, and elevates Sir2, PrxIII, Trx2, and CGRP in connection with contused brain and spinal cord [42, 45, 90, 91]. Furthermore, Dohi et al. have demonstrated that treatment with hydrogen-rich water reverses the expression of genes involved in oxidative stress, carbohydrate metabolism, and neuroinflammation following a traumatic brain injury [43]. Moreover, treatment with hydrogen-rich saline controls the inflammatory process in the brain tissue of rats with a traumatic brain injury by reducing the levels of proinflammatory cytokines (TNFa, IL-1b, and HMGB-1) as well as the number of inflammatory cells (Iba1) and inflammatory metabolites (Cho) [88, 89]. In addition, H2 molecules apparently can even suppress reactive astrogliosis, which is linked to oxidative injuries in the spinal cord [44]. In the contused spinal cord of rats, hydrogen-rich saline remarkably attenuates the local release of proinflammatory cytokines and the production of specific markers (STAT3, p-STAT3, and GFAP) as expressed by astrocytes and suppresses astrogliosis [44].

Overlevende efter en traumatisk hjerneskade oplever ofte kognitiv svækkelse, herunder nedsat læring og hukommelse. Disse mangler kan reverseres via behandling med brintberiget saltvand, sådan som det indikeres via forbedret kognitiv performance i Morris’ vandlabyrint efter milde traumatiske hjerneskader ved forekomst af brintberiget saltvand [91]. Brintberiget saltvand kan forbedre kognitive mangler efter traumer ved at opretholde synaptisk plasticitet. Hos rotter med en traumatisk hjerneskade øger brintberiget saltvand i væsentlig grad niveauet af hjerneafledt neurotropisk faktor (BDNF), calcium-/calmodulinafhængig proteinkinase II, synapsin I og cyklisk AMP-responselementbindende (CREB) protein i hippocampus [91]. Disse molekyler indgår i medieringen af synaptisk plasticitet og kognition. Generelt dæmper H2 ikke kun traumatiske hjerneskader via gassens almindelige antioxiderende, antiinflammatoriske og antiapoptotiske effekt, men dæmper også kognitive skader induceret af traumatiske hjerneskader ved at forbedre neuronal synaptisk plasticitet.

Subarachnoidalblødning (SAH)

Subarachnoidalblødning (SAH) er en voldsom cerebrovaskulær hændelse med høj morbiditet og mortalitet samt en dårlig prognose. Oxidativt stress er en nøglefaktor, som indgår i patogenesen af tidlige hjerneskader efter SAH. En antioxiderende behandling, der omfatter regulering af frie radikaler, er derfor effektiv til behandling heraf. H2 er en lovende terapeutisk metode til patienter med SAH på et tidligt stadie. Adskillige dyrestudier har vist, at behandling med brintberiget saltvand i væsentligt omfang dæmper den tidligere hjerneskade 24 timer efter SAH [9, 10]. På samme måde dæmper inhalation af 1,3 % – 2,9 % H2 i 2 timer efter en intracerebral blødning (ICH) hos rotter hjerneødem, opretholder BBB’s integritet, reducerer apoptose og neuroinflammation samt forbedrer den neurologiske funktion, med reduceret produktion af MDA, nitrotyrosin og 8-OHG i hjernen [46, 92]. H2 har imidlertid en neurobeskyttende effekt 24 timer efter SAH (akutte fase), men ikke efter 72 timer (forsinket fase) [46]. Det er interessant, at Manaenko et al. har observeret, at inhalation af 2 % H2 i 1 time i væsentligt omfang reducerer indholdet af vand i hjernen og forbedrer det neurologiske resultat, hvorimod inhalation af H2 i 2 timer ikke har nogen effekt 24 timer (akutte fase) efter ICH [93]. Hertil kommer, at 72 timer efter ICH er der større sandsynlighed for, men dog ikke i signifikant grad, at inhalation af H2 vil forbedre de neurologiske mangler. I modsætning hertil dæmper behandling med brintberiget saltvand det forøgede niveau af MDA, caspase-12 og caspase-3, og dæmper i væsentligt omfang hjerneskade og -ødem 72 timer efter SAH hos kaniner [49]. Baseret på disse rapporter kan H2 have en dosis- og tidsafhængig effekt i forbindelse med SAH. Herudover kan H2 forbedre cerebral vasospasme, som er en almindelig komplikation hos patienter med SAH. Hos rotter dæmper brintberiget saltvand neurologiske funktionelle mangler og morfologiske vasospasmer i a. basilaris efter SAH [47].

Nerve pain (Neuropathic pain)

Nervesmerter er en smertetype, der hører sammen med skade eller sygdom, som påvirker det somatosensoriske nervesystem, og som i væsentligt omfang påvirker patientens livskvalitet. Nervesmerter er besværlige og ekstremt udfordrende at behandle. Endnu mere vigtigt er det, at behandling med H2 kan være en terapeutisk tilgang til lindring af nervesmerter i forbindelse med forskellige patologiske tilstande, herunder opioidinduceret hyperalgesi, rygmarvsskader og postherpetisk neuralgi.

Postoperative hyperalgesia Remifentanil, which is a potent, short-acting, synthetic opioid analgesic, is used as an adjunct to an anaesthetic agent during surgery for pain relief. Hyperalgesia is a side effect of intraoperative analgesia administration with remifentanil. During the development of opioid-induced hyperalgesia, NMDA receptor NR1 and NR2B subunit membrane trafficking increases in the spinal cord, and this trafficking is mediated via the activation of glycogen synthase kinase-3b (GSK-3b) [51]. Zhang et al. have observed that remifentanil infusions induce rapid and prolonged mechanical and thermal hyperalgesia and promote NR1 membrane trafficking as well as GSK-3b activation in the dorsal root ganglion (DRG) [51]. Even more importantly, treatment with hydrogen-rich saline partially attenuates remifentanil-induced hyperalgesia without affecting the baseline of the nociceptive threshold, decreases the expression of inflammatory mediators (TNF-a, IL-1b, and IL-6), and suppresses NR1 membrane trafficking via inhibition of GSK-3b activity in the DRG in a dose-dependent manner [51]. In a rat model of incisional postoperative pain, the production of ONOO- increases in the spinal cord after the administration of remifentanil. ONOO- activates divalent metal transporter 1 without an iron-responsive element [DMT1(-)IRE] and induces abnormal iron accumulation, leading to the development of hyperalgesia [50]. Intraperitoneal delivery of hydrogen-rich saline can remove ONOO- from the spinal cord, protect against remifentanil-induced postoperative hyperalgesia, and attenuate DMT1(-)IRE activation and iron accumulation [50]. In addition, post-treatment with hydrogen-rich saline attenuates the postoperative mechanical and thermal hyperalgesia induced by remifentanil in connection with incisional pain in rats, prohibits NR2B expression and membrane trafficking from the intracellular pool to the surface pool, and blocks MnSOD nitration in the dorsal horn [52]. These results indicate that hydrogen-rich saline has an antihyperalgesic effect, possibly via the inhibition of oxidative stress and the GSK-3b-NMDA-MnSOD signalling pathway (Fig. 2).

Resultatet af et studie vedrørende nervesmerter induceret af L5 spinalnerveligering (L5 SNL) i en rottemodel viser, at intratekal infusion af brintberiget saltvand dulmer L5 SNL-induceret mekanisk allodyni og termisk hyperalgesi samt giver en relativt langvarig forebyggende effekt [25]. Ved kroniske forsnævringsinducerede skader i en anden rottemodel for nervesmerter forøger intratekal eller intraperitoneal injektion af brintberiget saltvand også i væsentligt omfang grænseværdien for mekanisk tilbagetrækning og latens for termisk tilbagetrækning for nervesmerter [11, 53]. I en musemodel for delvis iskiasnerveligering dæmper indtagelse af vand med H2 også i væsentligt omfang mekanisk allodyni og termisk hyperalgesi [12]. Udover oxidativt stress udløses smerter også af inflammation og aktivering af immunceller og glialceller i DRG [94]. Den analgesiske effekt af H2 kan muligvis også høre sammen med undertrykkelse af inflammation og oxidativt stress. Administration af H2 hæmmer aktiveringen af spinale astrocytter og mikroglia, reverserer overekspressionen af proinflammatoriske cytokiner (IL-1b, TNF-a og HMGB) og sænker niveauet af tyrosin-nitreret MnSOD, MDA, proteincarbonyl, 8-hydroxyguanosin (8-OHG), 8-OHdG, HNE og MPO (fig. 2) [12, 25, 53, 54]. I en rottemodel for ligering af iskiasnervestammen dæmper behandling med brintberiget saltvand smerter, sænker niveauet af proinflammatoriske cytokiner samt øger HO-1-proteinekspression og -aktivitet i DRG og rygmarven. Sideløbende reverseres effekten af H2 af HO-1-hæmmeren SnPP-IX og forstærkes yderligere af hemin og CORM-2. Chen et al. har derfor indikeret, at HO-1/CO-signalering indgår i den analgesiske og antiinflammatoriske effekt af H2 i forbindelse med nervesmerter [54].

Parkinson's disease

Parkinsons er den næstmest almindelige neurodegenerative lidelse, og den påvirker primært bevægelsessystemet. Sygdommen er karakteriseret af progressiv degeneration af dopaminerge neuroner i pars compacta i substantia nigra. Til dato har adskillige dyreforsøg og en håndfuld forsøg med mennesker vist, at H2 har en dæmpende effekt på udviklingen af Parkinsons.

Et randomiseret, dobbeltblindet, placebokontrolleret forsøg med 18 patienter fandt, at patienter, der drak brintberiget vand i 48 uger, opnåede en forbedring i den samlede UPDRS-score (Unified Parkinson’s Disease Rating Scale), mens UPDRS-scoren hos patienter, som drak vand med placebo, blev forværret. De foreløbige resultater indikerer, at indtagelse af brintberiget vand er sikkert og tåles godt. Hertil kommer, at det har en væsentlig indvirkning på Parkinsons sygdom [23]. Et andet klinisk forsøg med 20 patienter med Parkinsons, viste dog, at inhalation af 1,2 % – 1,4 % H2 i 10 min. 2 gange om dagen i 4 uger ikke havde nogle gavnlig effekt [95]. Og desværre forbedrede indtagelse af brintberiget vand i 72 uger heller ikke den samlede UPDRS-score hos 178 patienter med Parkinsons i forbindelse med et randomiseret, dobbeltblindet multicenterforsøg [96]. Selvom der ikke var nogen væsentlige forskelle i ændringen af scoren i gruppen, der drak brintberiget vand, og i placebogruppen i dette forsøg, påviste forsøget endnu en gang sikkerheden ved indtagelse af brintberiget vand [96]. Disse negative resultater kan skyldes graden af Parkinsons sygdom, UPDRS-scorens fase, H2-koncentrationen i det brintberiget vand eller varigheden af inhalation af H2.

In 6-hydroxydopamine or 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) rodent models of Parkinson's disease, administration of H2 is effective in inhibiting the development and progression of the disease [16, 55–57]. Further analysis has shown that the therapeutic effect of H2 in models of Parkinson's disease may be linked to the prevention of loss of dopaminergic neurons in the substantia nigra, as well as a decrease in the production of 4-HNE in the SN dopaminergic neurons and accumulation of 8-oxoguanine in the striatum (Fig. 3) [16, 55, 57].

H2 is one of the primary intestinal gases produced and utilized by the microbiota in the intestines. The gut microbiota is closely linked to Parkinson's [97]. This means that the brain-gut axis relationship during treatment with H2 has become an interesting topic. Matsumoto et al. reported that oral intake of hydrogen-rich water significantly increases gastric ghrelin expression in the stomach as well as plasma ghrelin levels in mice. Even more importantly, activation of the b1-adrenergic receptor is required for H2 to induce an increase in plasma ghrelin [58]. Consequently, studies have shown that the intake of hydrogen-rich water can improve the pathological process of Parkinson's disease via activation of the gastric ghrelin system (Fig. 3) [58]. Ghrelin knockout (KO) mice have been used to further validate the contribution of ghrelin in the mouse model of H2-treated Parkinson's disease. Yoshii et al. have observed that the administration of hydrogen-rich water significantly reduces the loss of dopaminergic cells in ghrelin-KO mice with MPTP insult [15]. In addition, the administration of D-Lys3-GHRP-6 is not effective in inhibiting the neuroprotective effect of H2 in ghrelin-KO mice with Parkinson's, unlike in wild-type mice [15]. They therefore suggested that ghrelin is not the sole factor associated with the H2-induced neuroprotective effect in connection with Parkinson's (Fig. 3) [15], and more studies are needed to investigate the stomach-brain relationship during treatment with H2. Mikako et al. have observed that the intake of the H2-producing precursor lactulose (a substrate for microbial fermentation) increases the concentration of H2 in breath and very minorly improves motor deficits in a rat model of Parkinson's [56]. It appears that the gut microbiota mediates gut-brain communication via microbial metabolites, including H2 [98], or H2 might modulate dysbiosis in the gut microbiota in connection with Parkinson's (Fig. 3).

Cognitive dysfunction (Alzheimer's disease)

Alzheimers sygdom er den mest almindelige neurodegenerative lidelse i verden, og er den primære årsag til demens hos ældre. Kvinder rammes oftere end mænd. Behandling med H2 kan være et muligt valg til patienter med denne sygdom, da det har været rapporteret, at H2 forbedrer den kognitive svækkelse i dyremodeller for denne sygdom. I mange kredse mener man, at ophobning af Ab-aflejringer er den grundlæggende årsag til Alzheimers. Intracerebral injektion af Ab1-42 hos hanrotter øger niveauet af oxidativt stress i hjernevæv, hvilket indikerer et øget niveau af MDA og 8-OHdG [59, 60]. I en transgen musemodel for Alzheimers har administration af H2 en beskyttende effekt [13, 14]. Indtagelse af brintberiget vand reducerer i væsentlig grad niveauet af MDA og forbedrer aktiviteten af T-SOD og GSH hos APP/PS1-mus [13]. I mus med 3 9 Tg Alzheimers sygdom regulerer en intracerebral injektion af PdH-nanopartikler (en H2-bærer med høj nyttelast) effektivt OH, reducerer Ab-generation og -aggregation, forbedrer mitokondrisk dysfunktion, reverserer de synaptiske mangler og hæmmer neuronal død i hjernen [14]. In vitro øger behandling med H2 det antioxiderende system i humane neuroblastome SK-N-MC-celler under Ab-stimuleret oxidativt stress via stimulering af AMPK og opregulering af downstream Sirt1-FoxO3a-aksen. Det forebygger mitokondrisk dysfunktion, hvorved celleoverlevelse ultimativt opretholdes (fig. 4) [61].

H2 improves cognitive impairment in rodent models of Alzheimer's, not only via the reduction of oxidative stress but also via the suppression of the inflammatory response in the brain. Administration of H2 effectively prevents extensive neuroinflammation in Ab1-42-challenged mice, with suppression of astrocyte activation and inhibition of proinflammatory factors (IL-1b, IL-6, and TNF-a) in the brain [59, 60]. In APP/PS1 mice, the intake of water with H2 reduces the levels of mRNA for IL-6 and TNF-a in the brain (Fig. 4) [13]. This indicates an anti-inflammatory effect of H2.

Det er interessant, at H2 har en kønsspecifik kognitiv fordel hos APP/PS1-mus uden at ændre Ab clearance og APP-bearbejdning. Oral brintberiget vand er effektivt til rumligt at forbedre læringsmangler, svækket hukommelse og østrogenniveauet (E2) i både hjerne og serum samt at øge ekspressionen af ERb, BDNF og TrkB i hjernen hos hun-APP/PS1-mus, men ikke han-APP/PS1-mus. Det er i overensstemmelse med nyere studier, som viser, at hun-3 9 Tg-AD-mus udviser mere udpræget amyloid plak, neurofibrillær sammenfiltring, neuroinflammation og rumlige kognitive mangler end han-3 9 Tg-AD-mus [99]. Disse data indikerer, at den mekanisme, der ligger til grund for den gavnlige effekt af H2, sandsynligvis er via E2-ERb-BDNF-signalvejen (fig. 4) [13].

Ved hjælp af en musemodel for præmaturt ovariesvigt induceret af zona pellucida 3 observerede He et al., at brintberiget vand i koncentrationen af Ab42 og APP, og ej heller øgede proteinekspressionen af NEP og IDE i cortex cerebri [13]. Disse fænomener indikerer, at H2 ikke indvirker på Ab clearance og APP-bearbejdning hos APP/PS1-mus. H2 kan med andre ord forbedre den kognitive svækkelse, men kan muligvis ikke hæmme eller reversere sygdomsprogression.

Udover forbedring af den kognitive dysfunktion ved Alzheimers dæmper H2 også den kognitive svækkelse, der induceres af operation, isoflurananæstesi, vaskulær demens, hypoksi, stråling, stress, status epilepticus og aldring [39, 78, 101–108], som ikke gennemgås her.

Mood disorders

Stemningsforstyrrelser (f.eks. angst og depression) er almindelige, men alvorlige mentale helbredsproblemer over hele verden, som har en negativ konsekvens for menneskers humør, dagligdag, arbejde og social kommunikation. De fleste af de tilgængelige angstmedicin- og antidepressivatyper er generelt sikre og effektive. Men > 50 % af patienterne opnår ikke komplet remission efter medicinsk behandling. Herudover har disse lægemidler bivirkninger og advarsler vedrørende brugen.

However, the precise mechanism of why H2 prevents the decline in E2 is unknown and requires further investigation. Overexpression of BDNF, TrkB, and synaptic proteins (postsynaptic density 95, synapsin I, and synaptophysin) has been reported [13, 14] in mouse models of Alzheimer's after treatment with H2, indicating that H2 potentially has a neuroprotective effect via the restoration of neuronal plasticity. In APP/PS1 mice, researchers also found that oral hydrogen-rich water did not lead to a decrease.

I de senere år har dyreforsøg vist, at H2 har en angstdæmpende og depressionsdæmpende effekt. Inhalation af 67 % H2 eller indtagelse af 4 ml oral brintberiget vand dagligt forebygger i væsentlig grad depressiv og angstlignende adfærd hos mus, der udsættes for kronisk mild stress (CMS) [17, 62], en gnavermodel for depression [109]. På den anden side undertrykker behandling med H2 stigningen i IL-1b, caspase-1 og ROS i hippocampus og den præfrontale cortex hos deprimerede mus [62]. Modsat reducerer administration af H2 også niveauet i serum af kortikosteron (CORT), ACTH, IL-6 og TNF-a hos CMS-udfordrede mus [17]. Hertil kommer, at H2 potentielt har en langvarig effekt i forhold til stressresiliens hos mus, da inhalation af H2 hos unge dyr i væsentligt omfang øger resiliens overfor akut stress i de tidlige voksenår [17]. Angst er en negativ psykologisk konsekvens af seponering af opioider, som kan dæmpes via administration af H2. I en morfinafhængig musemodel for naloxonaccelereret seponering fandt Wen et al., at administration af brintberiget saltvand ikke kun i væsentlig grad reducerer tabet af kropsvægt, hoppeadfærd og våd-hund-rysten, men også undertrykker angstlignende adfærd hos mus efter naloxonaccelereret seponering eller en spontan seponeringsperiode [63]. Behandling med brintberiget saltvand reverserer sågar hypothalamus-hypofyse-adrenal-aksens hyperaktivitet, som induceres af seponering af morfin, og hæmmer stigningen i plasma af niveauet af CORT og kortisol. I en musemodel for autisme forbedrer præ- og postadministration af brintberiget vand i væsentligt omfang valproinsyreinduceret angstlignende adfærd og sænker niveauet i serum af IL-6 og TNF-a hos museunger [64].

Med henblik på yderligere validering af den unikke rolle, som H2 har i forhold til emotionel regulering, er der udført et dobbeltblindet, placebokontrolleret studie med 31 voksne frivillige i alderen 20-49 år. Mizuno et al. observerede, at indtagelse af brintberiget vand i 4 uger i væsentlig grad reducerer (post- versus præbehandling) scoren for K6 (humør og angst), Chalder Fatigue-skalaen (sværhedsgraden af fatigue) samt Pittsburgh Sleep Quality Index (score for generel søvnighed og søvnighed i dagtimerne) [18]. Herudover var ændringerne i post-/præbehandlingsforholdet for K6-score og lavfrekvent komponenteffekt (aktivitetstest for sympatetiske nerver) væsentligt lavere i gruppen, der drak brintberiget vand, end i placebogruppen. Det indikerer, at administration af brintberiget vand kan være en effektiv metode til at højne livskvalitet og opretholde et godt helbred ved at forbedre humør, angst og det autonome nervesystems funktion i dagligdagen.

Mechanisms behind the H2-induced Neuroprotective effect

Several potential mechanisms may be involved in the neuroprotective effect that H2 exerts against neurological disease: antioxidant, anti-inflammatory, anti-apoptosis, regulation of autophagy, preservation of mitochondrial function, and preservation of BBB integrity.

The Antioxidant effect of H2

Mange sygdomme er knyttet til skader, der skyldes oxidativt stress, som stammer fra en ubalance mellem produktion og regulering af ROS. Den omfattende produktion af peroxider og frie radikaler kan beskadige cellekomponenter, herunder proteiner, lipider og DNA. H2 er anerkendt i vide kredse som en antioxidant via direkte neutralisering af cytotoksisk OH og ONOO- (fig. 5B), mens det ikke påvirker andre former for ROS, f.eks. superoxidanion (O2-), nitrogenoxid (NO) og hydrogenperoxid (H2O2), som alle har en fysiologisk rolle som signalmolekyler[3]. Aktuelt foreslår nogle forskere, at H2 måske har en antioxiderende effekt ved at fungere upstream for genereringen af ROS. ROS stammer primært fra elektrontransportkæden under processen med oxidativ fosforylering (fig. 5). Samtidigt har mitokondrisk kompleks I tæt homologi og et evolutionsforhold med [NiFe]-hydrogenaser. Toru Ishibashi foreslog derfor, at funktionen af H2 kan være som ensretter af det mitokondriske elektronflow for at konvertere ubiquinon til ubiquinol, og derved undertrykke omfattende elektronudsivning og ROS-generering [110]. Et nyt studie udført af Ishibashi observerede, at H2 undertrykker generering af superoxid i kompleks I og reducerer det mitokondriske membranpotentiale (DWm) [111]. Samtidig rapporterede Gvozdjáková et al. også, at indtagelse af brintberiget vand øger niveauet af produktion af mitokondrisk ATP via kompleks I- og II-substrat og øger niveauet af mitokondrisk oxideret coenzym Q (ubiquinon) i rotters hjertevæv [112]. De seneste resultater fra Professor Mas forskningsteam påviste, at den udviklingsmæssige aktivitet af H2 i eukaryotisk mitochondria er tæt forbundet med kompleks I, og aktiviteten forekommer omkring stedet for den komplet oxiderede ubiquinonbinding [113]. Herudover har de også observeret, at H2 i væsentligt omfang øger aktiviteten af mitokondrisk kompleks I under hypoksiske forhold [114]. Disse resultater forstærker derved hypotesen om, at H2 kan kontrollere generering af ROS ved kilden ved at fungere som en ensretter af det mitokondriske elektronflow i Q-kammeret (fig. 5A).

Apart from the gas's properties as a free radical regulator and rectifier of the mitochondrial respiratory chain, H2 can also increase the body's resistance to extensive oxidative stress by improving the levels of antioxidant enzymes and modulating the expression of redox-related genes. Treatment with H2 increases the activity of endogenous antioxidant enzymes in the brain and spinal cord, such as SOD, CAT, and GPx [29, 47, 115–117]. Additionally, H2 can regulate the expression of antioxidant genes. Through RNA sequencing and RT-PCR analysis, Chen et al. found that the expression of genes associated with oxidative stress, including Cox8b, Cox6a2, Cox7a1, Hspb7, and Atp2a1, is significantly downregulated after H2 treatment in mice with spinal cord injuries [42]. In a model of hypoxia/reoxygenation injury, the administration of H2 significantly promotes the expression of HO-1 and nNOS [36, 115]. Strong expression of HO-1 has also been reported after treatment with hydrogen-rich saline in a rat model of sciatic nerve trunk ligation [54]. Nrf2 is an essential factor for the endogenous antioxidant system, which plays multiple neuroprotective roles in connection with several neurological diseases by regulating the production of numerous cytoprotective proteins [118]. Yuan et al. have reported that the administration of hydrogen-rich saline increases Nrf2 expression, promotes translocation of Nrf2 from the cytoplasm to the nucleus, and increases the expression of downstream factors, such as HO-1 and NQO1 [7].

At the same time, increased expression of p-p38 MAPK, HO-1, and Nrf2 has been observed with H2 treatment in several in vitro and in vivo experiments [37, 119]. Therefore, treatment with H2 possibly induces adaptive responses to oxidative stress in the nervous system by evoking the Nrf2 antioxidant defence system.

Anti-inflammatory effect of H2

Omfattende studier har ført til solid evidens for, at H2 har en antiinflammatorisk effekt i forhold til forskellige sygdomme i nervesystemet. Behandling med H2 undertrykker i høj grad mikrogliaaktivering og sekretion af proinflammatoriske cytokiner (IL-1b, IL-6, TNF-a og HMGB-1) i dyremodeller for flere neurologiske sygdomme [5, 26, 120, 121]. In vitro hæmmer forbehandling med et H2-beriget medie i væsentlig grad astrocytters hypertrofi og proliferation, mildner ekspressionen af GFAP og svækker den øgede sekretion af proinflammatoriske cytokiner (IL-1b, IL-6 og TNF-a) i primære astrocytter efter H2O2-inducerede skader [44]. H2 hæmmer ikke blot ekspressionen af proinflammatoriske faktorer effektivt, men øger også de immunsuppressive cytokiner IL-10, TGF- b og YM-1 i hjernen [39, 88]. M1/M2-polariseringen af mikroglia er en vigtigt deltager i neuroinflammation [122]. Ning et al. har rapporteret, at behandling med H2 i markant grad hæmmer andelen af M1-mikroglia, men ikke påvirker M2-mikroglia in vitro [123]. Chu et al. har bemærket det fænomen, at antallet af M1-mikroglia reduceres i væsentligt omfang, og antallet af M2-mikroglia øges i væsentligt omfang i cortex efter behandling med brintberiget saltvand hos mus med hypoksisk/iskæmisk irritation [39]. Hertil kommer, at der opnås mindre amøboid/rund mikroglia og mere intermediær mikroglia hos mus, der får H2 i en høj koncentration[5]. Det tyder på, at H2 kan undertrykke neuroinflammation ved at hæmme mikrogliaaktivering og regulere mikrogliapolarisering.

The dysregulation of NF-jB has been linked to neuroinflammation. Some studies have indicated that H2 plays an anti-inflammatory role through the inhibition of NF-jB activation. In rodents with hypoxia/ischemia, treatment with hydrogen-rich saline significantly promotes the activation of AMPK, inhibits the activation of NF-jB, and leads to the downregulation of miR-21 and miR-210 in the brain [6, 39]. In a rat model of subarachnoid haemorrhage, treatment with hydrogen-rich saline reduces the protein levels of p-IjBα and nuclear p65, as well as increases the level of the cytosolic enzyme p65 [10, 48]. In a rat model of Alzheimer's, an intraventricular injection of hydrogen-rich saline inhibits the activation of JNK and NF-jB in the hippocampus [60]. Additionally, the ingestion of hydrogen-rich water significantly attenuates the activation of the NLRP3 inflammasome and reduces the expression of its downstream signalling molecules (cleaved caspase-1 and IL-1β) in the brains of female APP/PS1 mice [13]. Treatment with hydrogen-rich saline after a subarachnoid haemorrhage also lowers the protein expression of NLRP3, ASC, caspase-1, and IL-1β, and cleaves caspase-3 in the cerebral cortex of rats [10]. These results indicate that in addition to the modulation of NF-jB signalling pathways, H2 can also attenuate the inflammatory response by suppressing the formation of the NLRP3 inflammasome.
Oxidative stress can induce cell damage and promote inflammation in diseases of the CNS. It has been demonstrated that extensive ROS stimulate the expression of transcription factors such as NF-jB and promote the secretion of IL-1β via the activation of the NLRP3 inflammasome [124, 125]. In fact, the anti-inflammatory effect of H2 usually parallels its antioxidant effect [26, 126]. Therefore, the anti-inflammatory effect of H2 may also be due to the mechanism of alteration in gene expression caused by ROS.

The Antiapoptotic effect of H2

Udover den antioxiderende og antiinflammatoriske effekt af H2 har gassen også en antiapoptotisk effekt i forhold til nerveskader. H2undertrykker ekspressionen af Bax, caspase-3 og andcaspase-12, fremmer ekspressionen af Bcl-2 og BclxL samt øger Bcl-2/Bax-forholdet i hjernen [9, 37, 42, 48, 101, 103, 107]. Herudover dæmper behandling med brintberiget saltvand i væsentlig grad tabet af motoriske neuroner samt hæmmer frigivelsen af mitokondrisk cytokrom c (Cyt c) og aktiveringen af downstream caspase-9 og caspase-3 [127]. Yderligere forskning har vist, at behandling med H2 i høj grad sænker ekspressionsniveauet af Akt, GSK3b, p-Akt og p-GSK3b i hjernevæv og cerebrale mikrovaskulære endotelceller (CMEC’er) efter hypoksi/reoxygenering [33]. Andre studier er kommet frem til lignende resultater, som viser, at behandling med brintberiget saltvand øger fosforyleringen af Akt og GSK3b efter en subarachnoidalblødning. Hertil kommer, at disse gavnlige effekter af H2 afskaffes af Ly294002, som er en selektiv hæmmer af PI3K-signalvejen [9, 33]. Disse data antyder, at H2 potentielt beskytter hjernen mod apoptose via Akt/GSK3b-signalvejen.

Regulation of autophagy

Some studies have recently revealed that H2 may exert a neuroprotective effect via the regulation of the autophagy signalling pathway. In a rat model of neuropathic pain, Wang et al. found that treatment with hydrogen-rich saline attenuates hyperalgesia and activates autophagy [11]. Additionally, an intraperitoneal injection of hydrogen-rich saline significantly upregulates Beclin-1, HIF1a, and BNIP3 mRNA and protein expression, downregulates p62 mRNA and protein expression, and increases the number of autophagosomes and autolysosomes in the spinal cord [11]. In a rat model of postherpetic neuralgia, intraperitoneal injection of hydrogen-rich saline relieves neuralgia and activates autophagy by upregulating the expression of LC3, Beclin-1, and p62 [128]. Furthermore, in connection with neonatal hypoxic/ischemic brain injuries, Bai et al. have reported that the administration of hydrogen-rich saline increases the LC3II/LC3I ratio, increases Beclin-1 protein expression, and lowers the levels of phosphorylated mTOR, Stat3, and ERK in the damaged cortex [40]. Studies have shown that H2 can not only promote autophagy but also inhibit it. In a rat model of vascular dementia, ingestion of hydrogen-rich water reduces the number of autophagosomes, which is accompanied by the downregulation of FoxO1 and Atg7 expression levels, attenuation of the LC3-II/I ratio, and upregulation of the p62 level [101].

Preservation of mitochondrial function

Produktionen af ROS sker primært i mitochondria, og normalt påvirkes den også. Forskere har indikeret, at brintberiget saltvand kan dæmpe neuronale skader, muligvis via bevarelse af mitokondrisk funktion. Cui et al. observerede, at brintberiget saltvand sænker graden af mitokondrisk hævelse og ultrastrukturel forstyrrelse, opretholder den mitokondriske membrans integralitet samt bevarer tabet af DWm og Cyt c-frigivelse i hippocampus hos rotter med en iskæmi/reperfusionsskade [35]. I musemodeller for amyotrofisk lateral sklerose dæmper brintberiget saltvand frigivelsen af mitokondrisk Cyt c, genopretter aktiviteten i kompleks I og IV, undertrykker dannelsen af ROS i mitochondria samt fremmer produktionen af mitokondrisk ATP [127]. Mekaniske skader beskadiger mitochondrias struktur og funktion. hvilket fører til mPTP-åbning (mitochondrial permeability transition pore) og ATP-tab i neuroner. Effekten af skaden reverseres dog af behandling med H2 [42, 43]. I humane neuroblastome SH-SY5Y-celler fremmer forbehandling med H2 mitokondrisk aktivitet, hvilket indikerer en stigning i DWm, cellulær ATP-koncentration og forbrugsrate for O2 [119]. mPTP er et af de direkte steder for ROS [129]. I dette studie kan den neurobeskyttelse, der ydes af H2, derfor muligvis knyttes til hæmning af mPTP-åbningen. Tidligere forskning indikerer, at aktiveringen af mitokondrisk ATP-sensitive K? (mitoKATP)-kanaler beskytter neuroner mod beskadigelse og dødsfald forårsaget af iskæmi/reperfusion [130]. Zhou et al. gættede på, at brintberiget saltvand også aktiverer mitoKATP-kanalerne, da den gavnlige effekt af brintberiget saltvand på iskæmi/reperfusion i rygmarven delvist reverseres af 5-hydroxydcanoat, som er en selektiv mitoKATP-kanalantagonist [116]. I mellemtiden er der behov for yderligere validering af forholdet mellem H2 og mPTP- eller mitoKATP-kanalerne.

Preservation of the BBB

Blod-hjerne-barrieren (BBB) er en yderst selektiv og organiseret struktur, som består af endotelceller, pericytter, astrocytter, neuroner og ekstracellulær matrix. Den er også kendt som den neurovaskulære enhed, og den anses for at være centralnervesystemets portvagt [131]. Forstyrrelse af BBB forårsages primært af iskæmi/reperfusion, spontant hypertensivt slagtilfælde og mekaniske traumeinducerede nervesystemskader, og man har observeret, at H2 dæmper BBB-dysfunktion [73, 82, 88, 90]. Takeuchi et al. fandt, at oral indtagelse af brintberiget vand reducerer antallet af 8-OHdG-positive celler og kar med blodunderløben albumin i hippocampus hos rotter med tilbøjelighed til spontant hypertensivt slagtilfælde [73]. Matrixmetalloproteinaser (MMP’er) er vigtige faktorer i forhold til forstyrrelse af BBB. Hos dyr med traumatiske hjerneskader kan indtagelse af brintberiget vand potentielt have en beskyttende effekt i forhold til ødem og forstyrrelse af BBB via undertrykkelse af faldet i niveauet af AQP-4 og MMP2 og hæmning af stigningen i niveauet af HIF-1 og MMP-9 efter kortikal påvirkning [43]. Herudover spiller hjerneafledte CMEC’er en vigtig rolle i forhold til BBB, og behandling med H2 hæmmer CMEC-apoptose efter hypoksi/reoxygenering [33].

Conclusions and perspectives

Det er rapporteret, at H2 har flere biologiske egenskaber, herunder antioxiderende, antiinflammatorisk og antiapoptotisk effekt, samt at det beskytter mitochondria og BBB. Oxidativt stress modulerer ekspressionen af mange forskellige gener, som påvirker mange biologiske reaktioner, f.eks. apoptose, autofagi og den antiinflammatoriske respons. Det betyder, at den antioxiderende effekt af H2 kan være gassens mest grundlæggende egenskab. For indeværende er der to hypoteser vedrørende mekanismerne bag den antioxiderende effekt af H2. Den generelt accepterede hypotese er, at H2 direkte reagerer med OH og ONOO-, dvs. den traditionelle ‘‘reguleringsteori’’. Andre mener, at H2 undertrykker genereringen af ROS ved at fungere som en ensretter af det mitokondriske elektronflow. Selvom flere nyere udgivelser indeholder noget præliminær evidens for den nye hypotese, er der behov for mere overbevisende evidens.

Den neurobeskyttende effekt af H2 er blevet påvist via en stadigt voksende mængde evidens fra dyreforsøg. Gassens virkning også blevet rapporteret i mange kliniske forsøg, især vedrørende cerebral iskæmi, følger efter hjertestop og Parkinsons sygdom. Selvom den beskyttende effekt af H2 hurtigt har udviklet sig fra mange fundamentale studier til præliminær klinisk anvendelse i de senere år, er der behov for mere arbejde vedrørende de kliniske anvendelser. Utallige studier har vist, at H2 ikke har en toksisk effekt, men der er blevet rapporteret bivirkninger i enkelte tilfælde, herunder diarré, halsbrand og hovedpine [84, 132]. Hertil kommer, at Wang et al. observerede, at t H2-supplementering øger aktiviteten af hydrogenase i Helicobacter pylori i væsentlig grad. Endnu vigtigere er det, at højere hydrogenaseaktivitet og hydrogenmetabolisme i H. pylori kan inducere tarmkræft ved at fremme translokation af den carcinogene faktor CagA ind i værtsceller [133]. Det indikerer, at H2 potentielt kan øge patogeners patogenitet og fremme udviklingen af sygdomme i særlige tilfælde. Det betyder, at indikationerne for og de potentielle bivirkninger af H2 ikke kan ignoreres ved klinisk brug.

I nogle kliniske forsøg med Parkinsons sygdom havde administration af H2 ikke nogen gavnlig effekt, hvorfor den beskyttende effekt af H2 på en eller anden måde er begrænset under visse patologiske forhold. Som tidligere nævnt svinger distributionen af H2 i væv og organer i henhold til administrationsmetode og påvirker derved gassens biomedicinske effekt. Det tyder på, at den begrænsede effekt af H2 kan hænge sammen med koncentration, varighed og administrationsmetoden for H2 foruden sygdomsstadiet. Der er derfor behov for yderligere studier til undersøgelse af farmakokinetik og forholdet mellem dosis og effekt for H2, og derefter udvikling af mere effektive leveringsmetoder for H2.

Overordnet set, og på trods af de begrænsede bivirkninger og negative resultater, der er rapporteret af nogle studier, kan H2 potentielt være en lovende ny behandlingsmodalitet for adskillige sygdomme i nervesystemet.
